Trophies are the PS3 equivalent or 360's Achievements. Basically, things like "get 10 headshots in a row" or "beat the game on Hard mode with less than 5 deaths" etc.
Only thing is very few games have them right now. Most game developers probably won't waste their time making patches for games that are old now, since everyone's already bought the games and they have their money. (Not calling them cheap, it's just common sense)
Not all games are like this, though. Just today I patched Uncharted and now all the installed "Medals" it had have been converted for trophy support.
Also, all new games should have trophy support since it wouldn't need patching since the games are still in development.
