for mac users
if you are on a Mac, the only program that I have found that works is ffmpegx and only with these settings:
Use x264 encoder to encode into H264. The other H264 encoders don't work
In the Options tab, enable CABAC and Trellis. Set AVC to level 4 and Quantisiser to between 22 and 40.
i prefer to enable the decode with QT option as well because I have had some problems with video before and I figure this is one way to prevent future issues. I hope.
